Dr. James "Jay" Stevens earned a Bachelor of Arts in Zoology from UNC-Chapel Hill and a Doctor of Medicine from East Carolina University School of Medicine. He completed a Family Practice Residency at the Fairfax Family Practice Program at the Medical College of Virginia and the first Sports Medicine Fellowship at the University of North Carolina, Chapel Hill in 1991. He is board certified in both Family Practice and in Sports Medicine. He founded and currently serves as President of Carolina Family Practice & Sports Medicine with locations in Cary, Raleigh, and Holly Springs, NC. He serves as Head Medical Team Physician for the National Hockey League’s 2006 Stanley Cup Champion Carolina Hurricanes, a position he has held since 1997. In addition, he is Head Company Physician for the Carolina Ballet, and he provides sports medicine services to many local high schools.
His commitment to helping his patients create healthy lifestyles for long-term wellness, along with his desire to provide the highest quality health care services, has motivated Dr. Stevens to establish Essential Health & Wellness (EHW). EHW offers its patients a comprehensive health care service tailored around their busy schedules with the goal of optimizing their performance and longevity.
Dr. Stevens is also a member of, and certified by, the American Academy of Anti-Aging and Regenerative Medicine, the American Academy of Family Practice, and the American Medical Society of Sports Medicine.
Dr. Stevens lives in Cary with his wife, Dr. Patience Stevens, and their three sons Eric, James, and Timothy. He is an avid surfer and triathlete.

Born and raised in Asheville, North Carolina, it does not surprise many that Dr. William Haas has established himself as a leader in the fields of integrative and functional medicine. Not afraid of breaking the mold, Dr. Haas earned both his medical and business degrees from East Carolina University, one of the nation’s top primary-care training programs. He is board-certified in family medicine and attended the University of Arizona for his Integrative Medicine fellowship, where he later served on faculty. In addition to his integrative medicine training, Dr. Haas is a candidate for certification with the Institute for Functional Medicine.
Currently, Dr. Haas practices integrative medicine and serves as the Chief Medical Officer for Essential Health & Wellness. Additionally, he operates as the Vice President for Clinical Services at Essential Health Solutions.
Prior to his career as a physician, Dr. Haas worked as strength coach and personal-trainer, holding the highest certification with the National Strength and Conditioning Association. Previously an avid triathlete, Dr. Haas now enjoys competing in kettlebell sport competitions. Fitness and nutrition remain central to both his personal and professional practice.
As lead physician in the North Raleigh office of Essential Health and Wellness, Dr. Elizabeth Sierakowski is committed to an atmosphere of warmth and the pursuit of excellence in primary care. These qualities help patients achieve whole and lasting wellness through optimization of the body and integration of the mind and spirit.
Dr. Sierakowski earned both her Bachelor of Science in Biological Sciences and Doctor of Medicine from the University of Connecticut. She completed the Integrative Medicine in Residency during her Family Medicine training at the University of Arizona Alvernon, and is board certified in Family Medicine. She is Fellowship trained in Integrative Medicine through AzCIM and is pursuing Fellowship in Anti-Aging, Regenerative, and Functional Medicine through A4M, anticipating board certification in both Integrative and Functional medicines. She carries certificates in functional endocrinology, mitochondrial health, bioidentical hormone replacement therapy, and an array of integrative modalities.
Before joining EHW, Dr. Sierakowski was influenced by her work as a hospitalist, emergency, and primary care physician in the great American West, and as a pediatrician in a New England coastal private practice. She is a member of the American Academy of Family Practice and the American Academy of Anti-Aging and Regenerative Medicine. Professionally, her interests include preventive wellness care of children and adults, addressing exhaustion and burnout, and gut microbiome rehabilitation. She and her husband Jonathan enjoy traveling, adventures in the kitchen, and playing with their Goldendoodle pup Zelda.
Dr. Shearer is originally from Martinsville, VA. He attained a bachelor’s degree in Health Sciences from James Madison University and received his medical education from Virginia College of Osteopathic Medicine.
He completed an Internal Medicine Residency with University Hospitals Richmond Medical Center in Richmond Heights, OH followed by a Fellowship in Addiction Medicine through Larkin Community Hospital in South Miami, FL. Upon completion of his fellowship training, he served as a hospitalist for Mercy Hospital in Ardmore, OK and then as the Medical Director at the Good Shephard Community Clinic in Ardmore, OK.
He has completed a fellowship in Metabolic and Nutritional Medicine as well as a Weight Loss Certification through the Metabolic Medical Institute. He currently holds active board certifications with the American College of Osteopathic Internist, Obesity Medical Association, and American Board of Anti-Aging and Regenerative Medicine.
